Chronic Care Connection
Chronic Care Connection is our medical education program where parents of children with special needs teach health care professionals about critical care factors for children and their families.
We recognize the importance of family-centered care. Chronic Care Connection knows families facing the challenges of a chronic pediatric illness need three essential ingredients to live successfully within the community:
- A pivotal physician.
- Access to community resources.
- The highest quality of life possible for the child.
Through our family-centered care philosophy, Chronic Care Connection offers families positive experiences through life-long management of health care issues and encourages family partnerships with health care professionals.
