Data and research are essential to driving the link between social determinants of health and health outcomes.
ProMedica’s leading work as a health provider in the social determinants of health space is producing some significant proof points that can serve as a national model to drive transformational lasting change in the way we invest in health.
In 2014, ProMedica began screening patients for social determinants factors and in time, moved to connecting those patients to the appropriate interventions. Over time, we began to find that these proactive interventions could become a model of preventable treatment, and decrease future healthcare costs and utilization, while increasing healthier and more effective outcomes.
Food as Medicine: Connecting hungry patients to Affordable, Healthy Food
Medicare patients reduced Emergency usage by 18%, re-admit by 5%, and medical cost by 6%.
Medicaid patients reduced Emergency usage by 11% and re-admit by 1%.
Pathways HUB coordination in low birth weight reduction resulted in cost savings of $3.36 for 1st year of life and $5.59 long-term for every $1 spent on the initial intervention.

Results like these were made possible by ProMedica’s innovative investment in Socially Determined, a technology start-up who works to integrate social risk insights with clinical, claims, programmatic, and screening data.

Further, ProMedica and Socially Determined have partnered to create the Ebeid Data Nerve Center. By integrating ProMedica’s clinical excellence and deep data structures with Socially Determined’s powerful analytic capabilities, the Ebeid Data Nerve Center focuses on the needs of specific patient cohorts that could benefit from standardized interventions designed to improve health care outcomes.
This initiative disrupts traditional structures of population health management, and it creates a new, innovative model of precision population health. By doing so, ProMedica continues to strive towards improving health and well-being for ProMedica patients and a reduction in overall cost.
In August 2015, ProMedica and The University of Toledo College of Medicine and Life Sciences signed a watershed Academic Affiliation agreement to transform medical education, research and clinical care in northwest Ohio. Spanning 50 years and encompassing a financial investment of $2.5 billion, the Affiliation will build a legacy model of health care that will benefit residents of our region for generations to come.
