How do you get to ProMedica Fostoria Community Hospital? Get directions and view a campus map.
Where do you check in? From the parking area, use the main entrance and proceed to the information desk. Our volunteers will direct you to the appropriate area, where staff will help you with paperwork to begin the admission process.
What do you need to bring?
- A photo ID and your insurance card
- Items you need for daily function, like dentures, glasses, and hearing aids
- Robe and/or slippers, if you want them
How and when will you be discharged? We know you want to get home as quickly as possible. But we need to make sure you’re healthy enough first. When your doctor is satisfied with your progress and knows you can safely return home, or move to another care facility, he or she will begin the process.
Once your paperwork is complete, your nurse will let you know when you’re ready to leave the hospital. The person handling your discharge will help you arrange for any continuing care you may need. Learn more about continuing care services at ProMedica.
Don’t forget to collect your personal belongings when you leave. And be sure you understand your doctor’s instructions for medications and follow up.
