Can you leave information for people who call to inquire about you? If you give us permission to inform callers, our operators can state your room number, admission date, and general medical condition (poor, fair, good, or critical). Call patient information at 419-783-6955.
When can you leave the hospital? You’ll be discharged when your doctor is satisfied you’ve recovered enough to go home or to another care facility. If your medical team decides you need continued services to help you recover, like physical therapy, home health care, or home medical equipment, they will be arranged. Call 419-783-6955 ext. 5226 to speak to a discharge planner. Learn more about continuing care services at ProMedica.
What if you need someone to drive you home? Depending on your procedure, you may not be able to drive yourself home. If you do not have a way to get home, ask your nurse or discharge planner to help you arrange transportation.
