Update Insurance Info

Have you recently changed insurance companies or enrolled in a new plan? You can update your insurance information with ProMedica through our convenient online form. Please fill this form out as completely as possible and then click “submit.” You can find all of this information on your insurance card.

Your updated insurance information will be sent to one of our customer service representatives and, if needed, your account will be billed to your insurance.
