CMS issued Provincial House a Five-Star Quality Rating System ranking. The Five-Star ranking is the highest overall rating possible based on three key areas related to quality of care: health inspections, quality measures and staffing.

Our newly renovated facility has 117 beds in semi-private rooms. We provide a full continuum of health care services, so you or your loved one can be confident that the care you receive is consistent and of the highest quality. Because we are located near ProMedica Bixby Medical Center, our residents have easy access to general and acute hospital care and emergency services if they are needed. We can coordinate a wide variety of medical services, including hospice care, pain management clinics and dialysis treatments. We can also arrange visits with specialty physicians, including dentists, optometrists and podiatrists within our facility as needed. Provincial House is Medicare and Medicaid certified, and we accept most private insurances.
All of the coordinated care we provide is made possible by our skilled, caring staff. Helping you or your loved one learn to manage the ups and downs of a medical condition is truly a team effort. Our medical director specializes in skilled rehabilitation and long term care and oversees the facility. We have a number of professional therapists providing physical, occupational and speech therapy, as well as compassionate nurse practitioners, registered nurses, LPNs and nurses’ assistants who provide care. We also employ a number of social workers experienced in geriatrics. Finally, our registered dietitians can assess you or your loved one’s nutritional needs to promote health and healing.
Well-being is more than just a matter of physical health care. Having a fulfilling social life and being able to spend time with family and friends make a world of difference in how your loved one feels. At Provincial House, we promote wellness not only through our excellent health care, but also through individual and group activities such as barbeques on the outdoor patio, holiday celebrations and decorating contests, and religious services offered for various denominations. We also organize trips to local restaurants, community events, picnics, and sightseeing tours for viewing fall colors and holiday decorations. We love to meet our residents’ family members and strongly encourage families and friends to participate in activities at the facility. Additionally, we also have a private room available for birthday parties, anniversaries or special family gatherings.
Rehabilitation and Therapy
Our rehabilitation and therapy services aim to help our patients function at their highest potential after an illness, injury or disability. Skilled Nursing Care
For those whose physical conditions keep them from being able to care for themselves, we provide skilled nursing care 24 hours a day. Our nursing team works collaboratively with physicians and therapists to help your loved one manage complex medical conditions. For instance, we can help your loved one recover from surgery, stroke, or a cardiac condition. We are also here for any other medical condition that requires long-term care.
The Provincial House wound care team uses the most advanced treatments available to treat acute and chronic wounds. Often, conditions such as diabetes, poor circulation, or other medical problems can create wounds or pressure sores that require the care of a specialist. If you or your loved one has a hard-to-treat wound, our wound care nurses will design and oversee an individualized course of treatment.
