Medical research has shown that patients recover more quickly and experience better outcomes in the privacy and comfort of their own homes. The home care programs of ProMedica Health System provide skilled nursing care, private duty home health care and hospice. The coordination of additional services such as home infusion therapy pharmacy and medical equipment (including home respiratory services) is available ProMedica Home Infusion Pharmacy and ProMedica Home Medical Equipment.
Skilled Nursing Care Specialty Programs
- Wound care – Our certified experts will team with your physician to provide advanced wound care treatments that promote healing and reduce pain.
- Ostomy care – Certified wound, ostomy and continence nurses will assist you with products and lifestyle management skills.
- Restorative care – Our team of rehabilitation therapists and expert nurses will design a home exercise program to meet your needs following orthopaedic surgery or de-conditioning.
- Disease management – Our specially-trained nursing teams provide focused teaching regarding symptom control, prevention of complications, nutrition and lifestyle changes for conditions such as chronic obstructive pulmonary disease (COPD), diabetes, heart disease and others.
- Maternal and child health – Nurses with specialized training provide a full range of services during pregnancy and following delivery, including hydration infusions, skilled assessments, breast feeding techniques and infant care instruction. Pediatric nurses care for children with special medical needs and provide support and education to families.
A physician referral is required for home care services.
Disease management programs assist our patients and families to:
- Improve understanding of their disease processes/enhance education
- Decrease unnecessary use of health care resources
- Decrease re-hospitalization and use of emergent care
- Improve care coordination
- Identify problems early, intervene and avoid relapse of symptoms
Disease management programs currently include:
- Chronic Heart Failure Program
- Chronic Obstructive Pulmonary Disease (COPD) Program
- Diabetes Program
- Total Joint/Rehab Program
Disease management provides an organized approach to the care of patients with chronic illnesses. Patients are instructed on medication teaching, diet or exercise training that will improve their individual health status. This patient-centered approach addresses behavior, knowledge and medical treatment and improves patient outcomes.
Action plans are utilized with patient teaching and employ a color coded alert system. The color coding is a visual teaching tool and identifies the following for the patient:
- GREEN – “all systems go” or no problems
- YELLOW – possible areas of concern, caution, pay attention to warning signs or monitor symptoms carefully
- RED – danger or stop and seek medical attention immediately
Remote monitoring or Telehealth is utilized to enhance the disease management education and patient outcomes. Telehealth, a monitoring system placed in patients’ homes, helps patients take charge of their health care.

Infusion Therapy Services
ProMedica Home Infusion Pharmacy provides a full range of home infusion services, aerosol and self-injectable medications to patients in all regions served by ProMedica Health System. From IV antibiotic therapy and pain management to medications routinely ordered that fall under a patient's medical benefit, ProMedica Home Infusion Pharmacy is available to coordinate pharmacy needs in conjunction with ordered medical care.

Private Duty
In medically suitable situations, private duty home health care (nursing home health aides, homemaker services or companions) can provide clients and families with a viable option for safely providing care at home – potentially delaying hospitalization or transfer to an institutional setting.
Patients, or a designated payer, will be billed directly on a fee-for-service basis.
Private duty home health services may be an appropriate option when:
- Patient is recovering from an injury, illness, surgery or birth and care is not medically necessary but extra support is desired.
- Patient’s health condition limits activities of daily living.
- Spouse or companion is incapacitated and assistance is needed in providing care or respite.
- Family chooses to care for the patient at home.
- Placement in a nursing facility is not immediately available.
- Caregiver responsibilities outside the home make it difficult to ensure that a patient’s needs are met.
- Patient needs weekly or monthly medication set-ups to ensure safe and appropriate medication dosing and administration.
- Patient requires a non-skilled task prior to surgery (i.e. – enema preparation, etc).
Personal Emergency Response System
Whether a person has a chronic illness, disability or is recuperating from a recent illness or surgery, personal safety is an issue that weighs heavily on everyone’s mind.
Philips Lifeline is a personal emergency response system that operates 24 hours a day, seven days a week to provide peace of mind for patients and their loved ones. At the touch of a button, patients can reach specially trained operators with access to current patient medical data and the names and telephone numbers of three patient-selected emergency contacts.
