Chronic Care Connection (formerly known as Project D.O.C.C.)
Our physicians at ProMedica Toledo Children’s Hospital recognize the importance of family-centered care. Under Chronic Care Connection, a medical education program at ProMedica Toledo Children’s Hospital, parents of children with special needs teach physicians about critical factors in the care of these children and their families.
Chronic Care Connection emphasizes that a family facing the challenges of a chronic illness needs three essential ingredients to live successfully within the community:
- a pivotal physician
- access to community resources
- the highest quality of life possible for the child
In addition, Chronic Care Connection encourages family partnerships with healthcare professionals, as physicians can help families understand their unique needs. Through a “family-centered care” philosophy, Chronic Care Connection offers families positive experiences through life-long management of healthcare issues.
