Mary Ellen Falzone Diabetes Center
At ProMedica, we are committed to improving the health and well-being of people with diabetes. As part of this commitment, we have created a diabetes center uniquely designed to improve the diagnosis and treatment of diabetes in our community. Named in memory of a young girl who passed away from complications from Type 1 diabetes, ProMedica Mary Ellen Falzone Diabetes Center is the region’s first facility dedicated to diabetes education and care.
Located on the North Campus of ProMedica Toledo Hospital, the Center provides pediatric and adult patients with the latest care, education, and many other services, in a convenient, centralized location. Patients are able to see an endocrinologist, learn how to manage their disease and have their prescription and durable medical equipment orders filled all in one visit.

Specialized Care and Services
ProMedica Mary Ellen Falzone Diabetes Center gives patients access to an expert, multidisciplinary team of endocrinologists, diabetic educators, dietitians, midlevel providers, and pharmacists. This qualified team provides patients with the tools and encouragement they need to manage their disease. Some of the services provided include:

Diabetes counseling: Evaluation and education programs are available to help you learn to live a full and healthy life while keeping your diabetes under control.
Insulin therapy and glucose monitoring systems: Diabetes supplies as well as other medical goods and services are available on site through The Pharmacy Counter.
Medication therapy management: Receive one-on-one counseling with The Pharmacy Counter’s expert pharmacist to better understand your medications, including what they do and when to take them.
Primary care providers: If you need a primary care physician, schedule an appointment with a doctor, advanced practice provider or one of our family medicine residents from the ProMedica Toledo Hospital Family Medicine Residency program.
Laboratory services: Blood draws and testing available, including for A1C.
Construction for the 55,000-square-foot ProMedica Mary Ellen Falzone Diabetes Center began in early 2013 and was completed and ready for occupancy in February 2014.
The two-story, environmentally friendly building is Leadership in Energy and Environmental Design (LEED) certified, meaning that the building uses 20% to 30% less energy a year than a “standard” design.
ProMedica foundations direct charitable contributions from our friends in the community on behalf of specific programs and services. Our hospitals and foundations are not-for-profit organizations, and all contributions will be used to provide ongoing counseling and education and to expand upon those programs and services that benefit patients and families coping with diabetes.
