Phillips Telehealth Monitoring System
Home is where you want to be. With ProMedica Home Health Care’s Phillips Telehealth, we can help you do just that. Telehealth is a monitoring system that our nurse installs in your home and shows you how to use. You take your vitals every day and put them into the machine. A specially trained nurse monitors your health. Many patients say they eat better and take their medications on time because of their regularly scheduled Telehealth monitoring sessions. ProMedica Home Health Care nurses have been able to keep patients out of the hospital and in their home with Telehealth. Our team of rehabilitation therapists and expert nurses have the knowledge and experience needed to create a custom home exercise program to help patients recover after injurt, illness, or orthopaedic surgery and adjust to a less demanding lifestyle, if that’s what’s needed.
Our interdisciplinary team helps patients avoid complications, control symptoms, make nutrition and lifestyle changes, and more. These experts routinely work with patients suffering from chronic obstructive pulmonary disease, diabetes and heart disease, to name a few.
Maternal and Child Health
If you’re pregnant or have just delivered, ProMedica’s specialized nurses can be of great help with hydration infusions, skilled assessments, breast feeding techniques, and infant care instruction. In addition, pediatric nurses, physical therapists, and speech therapists take expert care of children with special needs, while giving families support and the knowledge needed to face possible challenges.

In some situations, private duty health care can help provide care at home, which can potentially delay hospitalization or transfer to an institutional setting. ProMedica’s nursing home health aides, companions and homemaker services are often a welcome option.
Private duty home health services may be an appropriate option when:
- A patient is recovering from an injury, illness, surgery, or birth and care is not medically necessary but extra support is desired.
- The patient’s health condition limits daily activities.
- A spouse or companion is incapacitated and assistance is needed in providing care or respite.
- The family chooses to care for the patient at home.
- Placement in a nursing facility is not immediately available.
- Caregiver responsibilities outside of the home make it difficult to ensure that a patient’s needs are always being met.
- A patient needs weekly or monthly medication set-ups to ensure safe and appropriate medication and administration.
- A patient requires a nonskilled task prior to surgery (for example, enema preparation, etc.).

Personal Emergency Response System
Personal safety is an issue that can weigh on the minds of patients and caregivers alike. Whether a patient is suffering from a chronic illness, a disability or recuperating from a recent illness or surgery, Philips Lifeline may be the answer.
Philips Lifeline is a personal emergency response system that operates 24 hours a day, seven days a week, to provide peace of mind. All a patient has to do is simply press a button to be connected with specially trained operators who have access to the patient’s current medical information, as well as the names and phone numbers of three patient-selected emergency contacts.
